Given COVID restrictions, this form is not yet confirmation of registration. We will follow up by Thursday morning to confirm after we understand the demand and capacity.

Service: Shabbat Shacharit
Location: Member backyard in Brookline (Amory area location, exact location to be provided upon registration confirmation)
Start time: 9:30 AM
Attendees:
- Maximum of 15 individuals
- Only attendees over Bar/Bat Mitzvah age
- No attendees with symptoms, confirmed cases of COVID-19, or exposure to others likely to have COVID-19 in the last 14+ days, absent a confirmed negative test
- All those who are at-risk of further complications from COVID-19 (including anyone with medical conditions outlined by the CDC or those over the age of 60) should speak with your medical professional to make your own decision - we rely on your judgment to keep yourself, and others, safe
- All attendees must sign up in advance, confirm their lack of symptoms, and agree to let WSM leadership know if they are symptomatic/confirmed in the next 2 weeks
